The clipboard is the slow part
Paper costs you time twice: once at the counter, and again in the back office.
A patient fills out four pages at the counter with a pen that keeps walking off. Later, somebody reads the handwriting, types the answers into your system, scans the pages, and files the paper. When that person is out for the week, the stack waits for them.
The patient fills out the same form on their own phone or computer before the visit, or on a tablet at the counter, and signs on the screen. FormRetriever collects the form on its next check, every fifteen minutes by default, and files the PDF in the right folder on your server, named the way your staff expect, with the insurance card photo alongside. Nobody retypes a word or scans a page.
Filed the way your office already files
FormRetriever runs on a PC in your back office. You pick one question on the form, like which location or which provider, and each answer gets a folder. From then on, every PDF:
- Gets the name your staff expect. Date, submission number, and last name, in whatever order you like.
- Lands in the folder for the answer the patient gave. On that PC, or a share on your file server.
- Brings its attachments along. The insurance card photo is saved beside the form.
- Waits in a review folder if something doesn’t fit. A blank answer, a new one, an edited form: each comes with a note saying why, and the count stays on screen until the folder is cleared.
The first check collects every form already submitted, oldest first, so a form that’s been live for a year doesn’t start from today.

Where your patients’ answers live
- On a forms platform built for HIPAA. Clinic Forms runs on HIPAAtizer, and HIPAAtizer is on our subprocessor list with exactly what the platform can see.
- Then on your own computers. FormRetriever only reads from HIPAAtizer, and the PDFs are written to your PC or your file server. Nothing runs on our side, and we have no console into your forms.
- Nothing is written over. Two documents never share a file name, and nothing in your HIPAAtizer account is edited, archived, or deleted.
- We run our own paperwork here. Every form on our Forms page lives on HIPAAtizer, including the business associate agreement our clients sign with us.

What does Clinic Forms cost?

Do you charge to convert our paper forms?
No. HIPAAtizer rebuilds each form by hand, and there’s no charge to you.
How many forms can we have?
As many as you need. Your site has no limit on forms or on submissions.
What can patients fill out the forms on?
Any device with a web browser: a phone, a tablet, or a computer. Patients sign on the screen. Send patients the link by text or email before the visit, add the link to your website, or hand a tablet across the counter.
Can patients attach a photo of their insurance card?
Yes. What they upload comes down with the form and is saved beside the PDF. That’s a switch per form, so you can leave attachments off where you don’t want them.
Where do the completed forms end up?
In the folders you choose, on a PC in your office or a share on your file server. FormRetriever sorts them by an answer on the form, such as which location the patient is visiting, and names each PDF the way your staff expect.
Is Clinic Forms covered by a business associate agreement?
Yes. The BAA you sign with us covers Clinic Forms, and HIPAAtizer, the platform the forms run on, has signed one with us.
Do we have to be a managed client?
No. Managed clients get FormRetriever installed and set up as part of the service. Everyone else installs FormRetriever with us on the phone, and setup is one screen.
